Created by Halcyon Homes, staffed by SN Services, and supported by Anderson Neurological & Developmental Services, The Leaving Hospital provides a warm, welcoming place where you can heal at your own pace. 


A first in Perth, this flexible and adaptable residential rehabilitation puts you and your loved ones at the centre of decision-making and your post-hospital recovery journey. The collaboration allows you to tailor services and supports based on your needs, preferences, and funding framework.

Stay for a Little While or as Long as You Need

Recovery looks different for everyone, which is why The Leaving Hospital offers both short-term and medium-term accommodation options. Whether you need a brief stay to regain strength before returning home or a longer period to rebuild independence, our thoughtfully designed living spaces provide the care and support you need.

  • Short-Term Accommodation

    Healing after a hospital stay is easier with the right support, and we’re here to provide just that. Our short-term accommodation is designed for those who:


    • Need a safe, welcoming place to regain strength before returning home.
    • Require medical support, rehabilitation, or assistance with daily activities.
    • Are waiting for home modifications or in-home care arrangements to be ready.

  • Medium-Term Accommodation

    For those who need more time to heal, our medium-term accommodation provides a calm and nurturing environment where recovery can continue at a steady pace. This option is ideal for those who:


    • Are recovering from major surgery, illness, or injury and need extra time to regain confidence.
    • Require ongoing therapy, nursing care, or rehabilitation before transitioning home.
    • Benefit from a peaceful, fully equipped space where they can focus on their well-being with compassionate support.

We specialise in complex care, ensuring your daily life seamlessly balances your clinical requirements. Our goal is to help you regain confidence by assisting with therapy programs, guiding you and your loved ones in using medical equipment, and supporting personal care and daily activities.

Continuing Therapy by Anderson Neurological & Developmental Services

Continuing therapy is vital for maintaining health, improving well-being, and restoring independence after a hospital stay. Through Anderson Neurological & Developmental Services, you can receive tailored rehabilitation programs designed to support your unique recovery goals.


Therapy plans may include on-site sessions in a state-of-the-art neuro gym, private sessions within the comfort of their accommodation, or outdoor therapy on the roof deck. To encourage long-term progress, you, your support workers, and family members can receive training to continue exercises at home or in the gym between scheduled sessions. Advanced rehabilitation is also available at Anderson’s Perth robotics gym, where specialised simulators enhance therapy outcomes.


Whether integrating with existing therapy or developing a comprehensive new program, Anderson ensures you receive the guidance and support you need to regain confidence and improve your quality of life.

  • Halcyon Homes

    Halcyon Homes is an NDIS Specialist Disability Accommodation (SDA) registered provider based in WA. With thoughtfully designed homes at 451 Beach, Halcyon offers private, self-contained apartments that prioritise comfort, independence, and support. 


    Unlike traditional rehabilitation facilities, each apartment includes a second queen bedroom, allowing loved ones to stay without sharing with unfamiliar occupants. At Halcyon Homes, the focus is on creating a safe, home-like environment where individuals can recover and regain independence at their own pace.

  • SN Services

    Specialist Nursing Services is a unique, for-purpose, comprehensive care agency dedicated to providing a range of at-home services to clients with complex medical and technological needs. Our highly trained team specialises in assisting those recovering from catastrophic injuries, ensuring both planned and unplanned care is delivered with expertise and empathy. 


    With a strong client-focused approach, SN Services works closely with you and your family to create a supportive environment that balances everyday life with essential clinical care.

  • Anderson Neurological & Developmental Services

    Anderson Neurological & Developmental Services operates from a state-of-the-art on-site neuro gym, offering cutting-edge rehabilitation for individuals recovering from neurological conditions. 


    Combining international expertise with advanced robotic therapy, their specialised programs are designed to accelerate recovery and sustain long-term progress. Their hands-on, personalised approach ensures that every therapy plan is tailored to meet the unique needs of each individual, helping them regain strength, mobility, and independence.

Your Funding Options

The Leaving Hospital offers flexible funding options to accommodate a variety of NDIS participants and funding arrangements.

  • NDIS Funding

    If you are new to the NDIS, you will probably have received a crash course on how NDIS funding packages work. There are two principles to be aware of. You will have:


    1. Your own individual funding package with a budget designed for your specific needs, which may change from time to time as your needs and circumstances change, and
    2. Choice and control to select your preferred service provider.

    With your individualised funding package, you can choose to enter into a Service Agreement with one or all of the collaboration partners OR select an alternative service provider(s). Your choice. You can also make changes to your team at any time if the fit isn't right for you.

  • NDIS Accommodation Funding

    The NDIS typically does not fund accommodation except in a few circumstances, such as:


    1. Medium-Term Accommodation: Up to 90 days for people transitioning from hospital or waiting for permanent housing. (From 3 October 2024, this funding must be included in your NDIS plan.)
    2. Specialist Disability Accommodation: This is an annual allowance granted to people who meet the NDIS criteria for housing to meet their needs specific to their disability. The NDIS allocates funds in your plan to subsidise your rent. We currently have a couple of remaining vacancies for SDA accommodation. See our SDA page for more details.
    3. Short-Term Accommodation: This funding is available on a case-by-case basis for up to 28 days for those with carers who may need a break. You can book short-term stays with us and benefit from the services offered by our collaborators, or you can bring your own support.

    From 3 October 2024, Medium-Term Accommodation funding must be stated in your funding plan. Your hospital NDIS Liaison, Social Worker, Support Coordinator, NDIS Planner or LAC should be made aware as early as possible that you will need this funding stated in your plan to be eligible. 


    For both MTA and SDA, there will be modest, affordable out-of-pocket rental expenses and utilities costs.

  • NDIS Daily Supports, Nursing & Therapies

    Your NDIS funding covers support for your disability-related needs. If you're new to the NDIS, your hospital team (social worker, NDIS liaison, or support coordinator) will help set up your plan. If you don’t have a plan yet, contact us—we can assist in creating one to support your transition from the hospital to our facility and then to your permanent home.


    If you already have funding, reach out to see if your plan covers our services or needs adjustments.


    Our transitional service offers private apartments with shared support, making it more cost-effective for the NDIS compared to at-home care. We’re committed to keeping your stay affordable while helping you gain skills that reduce your long-term support needs.


  • Private Insurance or Alternative Funding

    If you are privately funded or covered by another insurance scheme, our team can help you navigate your options. We’ll work with you to create a tailored package that aligns with your needs and budget, ensuring a seamless transition to The Leaving Hospital.


    SN Services and Anderson are registered with the Insurance Commission of WA, making it easier for you to access funding through alternative insurance schemes.

Daily Living Expenses – Meals, Laundry & More

In your fully equipped apartment, you’ll be responsible for your own meals, cooking, cleaning, and laundry. Depending on your NDIS plan, you may have funding for support with some or all of these tasks—your allowance will be tailored to your needs.


Enjoy the flexibility of eating what you want, when you want—whether cooking at home, ordering in, or dining out. The choice is yours.


For added convenience, we have on-site laundry facilities. If you're receiving assistance from SN Services, we can take care of washing your linens and clothes in our laundry, reducing noise and disruption in your apartment.
